Chords for Rounding The Cape?

Whilst my PC is out of action, can anyone help with the chords to Tol & Tol's Rounding The Cape please?

Thanks for these. The progression is quite simple but I just could not get that penultimate chord. C+4 it is.
Nearest I got was Gm7 and as Roy Walker said "it was good but its not right".
