I tried to Google the answer - no luck so far, but there is a site http://www.hopgnosiscovers.com which references an excellent image of the cover in question, as well as a couple of hundred others.

As well as the stereo issue, I also have a copy of the quadraphonic or quadrophonic issue of the album. Apart from some text about the quadraphonic system, the sleeve is the same as the stereo issue. Great website from Hipgnosis, by the way!
